Health Office Reminders

Absence From School Doctor Note - Absences of five (5) days or longer require a note from a physician prior to the school readmitting your child to class. All medical documentation should be submitted to the Health Office within five (5) school days of the absence.

Extra Clothes For Your Child - Please be sure to keep a spare outfit, including underwear, in your child's school backpack. In the event of a bathroom accident or mishap here at school, your child will be able to change his/her clothes and return to class. Additionally, we will not need to interrupt your day by having you bring a change of clothing to us!

Phone Contact Numbers - Please Inform the Main Office or Health Office if your home, cell, or work phone number changes. It is imperative that the school nurse can reach you if needed during the school day. Also, please let us know if any of the phone numbers of the persons you listed as emergency contacts change.

Physicals Exams - Please send in a copy of your child's annual physical exams after their doctor appointment. (The main office staff can make a copy for you.) Additionally, per Massachusetts state regulations, Grade 3 students MUST have an updated physical exam on file prior to starting Grade 4.
