Injuries and Student Physical Accommodations

A written letter/note from a medical provider is needed for the situations below. Medical notes are kept on file in the student's                                    school health record.

• Students who require a boot or cast, crutches, wheelchair, or any other medical device.
• Students who will need to use the school elevator for a medical issue.
• Students who cannot fully participate in all school activities, including gym class.
